In the Fall of 2009, the College of Arts & Sciences began developing standardized forms (i.e., editable PDFs), for many of its routine procedures, as part of its Communications initiative. Those procedures that apply to faculty and staff (and in some cases, students) are posted below. All can be completed online but, in most cases, they must be printed, signed, and submitted in hard copy (usually, directly to your Chair). Additional forms specific to the duties of a Department Chair can be found on the password protected eChairs site (see top left menu link).

This initiative is only for those procedures that take place within the College; as

much as possible, copies of forms for University-wide procedures will be posted here as well. Although A&S, of course, does not control these procedures, it will endeavor to ensure that these forms are also always up-to-date.


Retention and Tenure & Promotion

Tenure and/or Promotion Cover Sheet and Checklist (2 pages; Note: to appear in all retention, tenure, and promotion dossiers)

Sample Department Tenure & Promotion Committee Report

Teacher Course Evaluation (TCE) Summaries (to be included in all retention and tenure & promotion dossiers, for each Fall and/or Spring semester in which you have taught at UA. Note: Those involved in Spring 2009's pilot study of the online course evaluation should check back prior to the Oct. 1 deadline for submitting your dossier, by which time that data should also be loaded into this database.)

Note: Candidates applying for tenure and/or promotion must contact Yolanda Barnes in the Dean's Office (8-2530 or ywbarnes@as.ua.edu) to obtain the new External Review Letters Folder, which is to be inserted into your dossier at the appropriate place, and into which your Chair will add the solicited, confidential letters.


Compensation


External Supplemental Compensation Request (to be submitted for all external activities that each earn more than $500 of additional income for a faculty member; permission for internal supplemental is processed through the Chair)


Grants

Sabbaticals


Sabbatical Leave Request (a University-wide form)

Sabbatical Leave Report (to be submitted to Chair one month after returning from sabbatical)


Travel
(for requests made for support of faculty or student travel by A&S)


Request for Faculty Travel Funding from A&S (submit if requesting financial assistance from A&S)

Request for Student Travel Funding from A&S (submit if requesting financial assistance from A&S)

International Travel Request Form (submit at least two weeks prior to all international travel for work)

Cuba Travel Funding Agreement (for those participating in the College's Cuba Initiative)
