In the Fall of 2009, the College of Arts & Sciences began developing standardized forms (i.e., editable PDFs), for many of its routine procedures, as part of its Communications initiative. Those procedures that apply to faculty and staff (and in some cases, students) are posted below. All can be completed online but, in most cases, they must be printed, signed, and submitted in hard copy (usually, directly to your Chair). Additional forms specific to the duties of a Department Chair can be found on the password protected eChairs site (see top left menu link).
This initiative is only for those procedures that take place within the College; as
much as possible, copies of forms for University-wide procedures will be posted here as well. Although A&S, of course, does not control these procedures, it will endeavor to ensure that these forms are also always up-to-date.
Teacher Course Evaluation (TCE) Summaries (to be included in all retention and tenure & promotion dossiers, for each Fall and/or Spring semester in which you have taught at UA. Note: Those involved in Spring 2009's pilot study of the online course evaluation should check back prior to the Oct. 1 deadline for submitting your dossier, by which time that data should also be loaded into this database.)
Note: Candidates applying for tenure and/or promotion must contact Yolanda Barnes in the Dean's Office (8-2530 or ywbarnes@as.ua.edu) to obtain the new External Review Letters Folder, which is to be inserted into your dossier at the appropriate place, and into which your Chair will add the solicited, confidential letters.
